If possible, take item with you. If
not, find out when and how we will get the item.
Indicate this on the form. 3)
Offer to make a copy of the form for the donor. 4)
Fax a copy of the form to Cal Levi, EA at 602-495-4539. a.
If receipt of the item is after submission of the form, please send an
updated form indicating receipt. 5)
